Muelu eti split

Merged commits as proposed by Andrey. Did not move the macro in CMakeLists.txt. The idea is to move it into an extra file once the MueLu adapters ETI system has been migrated to the same ETI logic as MueLu and MueLu Interface. This will be done once these patches are approved and known to work as intended.
